Safety Issue

The job was completed, however while in clean utility replacing sprinkler head, ladder was leaned against the supply cart that had wheels on it. When he was up on the ladder the cart rolled and he pulled down the ceiling tile and its metal support. We had extra ceiling tile and he did replace and repair the damage.
