Took a digital scan of the tooth and built a new white porcelain crown. Placed porcelain crown with permanent dental cement. Checked the bite and made adjustments. The patient was happy.

Patient present at office for fillings upper right site corresponding #2#3#4 periodical was taken Dr numb area , removed decay and check if no remanent decay are on the walls after that used etch, bonding and composite to fill areas. also checked bite, adjusted bite and polish all restaurations.
pt present at office for fillings 11-MFD, 14-OBL, 15-OL, 30-OL decay was removed , bond light cured ,and composite Checked bite using blue paper, advised patient may need to come back for additional adjustments. pt used NITROUS AT 70/30
Patient was seen for a cleaning and check up by the doctor. He is scheduled to come back for one more cleaning this year.
