Male, adult patient, presented for new hygiene foundation appointment. Took xrays, periodontal charting, and intraoral pictures. Patient having pain on upper right tooth, big area of decay and needs extraction/bone graft. Patient also stated that all of his lower front teeth are very loose, will need extractions in the future. Patient will call to schedule.
