Good afternoon. My name is Mark Ray. Today I was at your home to perform your initial premium plus service. After my inspection, I determined to place two mouse bait stations in your basement, one next to the washer and dryer on the floor and another in the opening in the ceiling right over top of the electrical box. I also installed two mouse bait stations in your garage. I replenished the existing one behind your stove. And I also replenished the three existing large rodent bait stations on the exterior of your home. I also installed four termite monitoring stations. I also treated the exterior perimeter of your home, including windows and doors with a liquid product to prevent entrance and or infestation, and also to provide residual protection from all general pests. Removing all visible and reachable cobwebs from around the exterior of your home is also part of your quarterly service, however, today you had no cobwebs to remove. I did not notice any actual activity or evidence at
